His struggle to improve the living conditions for the patients takes place within the social and political history of 19th century Hawai'i, including the attitudes toward Hansen's disease that prevailed during that time. The first Hansen's disease patients were sent to Kalawao, a settlement east of Kalaupapa. In 1884, a transition from Kalawao to Kalaupapa occurred as the settlers were attracted to the drier and more pleasant environment of Kalaupapa.
